The only time I have seen the PSU fan spin, is when I start the PC up. Runs for about 2 seconds, then stops.

Normal. The better psu's only have the fan turn on when needed, and only then under a particular load. It turns on initially as a check.

It's done by load. Most of your psu's now a days, especially from evga and corsair, advertise how quiet their PSU's are and how it takes like 75% load or whatever before the fan will even both to come on because it has such great passive cooling built in.
