is the Fatality game card worth the extra cost?  Anyone have any opinions or suggestions? Does it improve gameplay over the xtreme gamer?

I will offer an opinion.
 
If you are going to use the extra features offered, the card is worth it. The card is a musician's dream.
 
If you are hoping to see any sort of significant FPS improvement over the XFi, there isn't one. If you were hoping for any improvement in audio quality, there is none. The two cards provide exactly the same signal to noise ratio, and the same frequency response. In testing, the gains provided by XRAM were 0 to 1 FPS.
